Anticoagulation Clinic

Health Begins Here.

Voyage Healthcare’s Anticoagulation Clinic offers dedicated monitoring services for patients taking Warfarin (Coumadin), a medication commonly referred to as a “blood thinner.” Our caring team of experienced nurses will assess each patient’s anticoagulation status and associated risk factors to develop a personalized treatment plan. We work closely with the patient to keep them safe and ensure the medication works as it should. Anticoagulation clinic services are provided at our CrystalMaple Grove, and Osseo locations.

Anticoagulation therapy is commonly used to prevent harmful blood clots from forming in patients with a history of:

  • Atrial fibrillation or other heart rhythm disorders
  • Blood clots or DVT
  • Mechanical heart valves
  • Pulmonary embolism
  • Stroke

Routine blood tests are an essential component of warfarin use.  These quick tests allow our healthcare team to measure the blood’s clotting ability by monitoring the INR level (International Normalized Ratio) on a regular basis.  Patients should expect to be seen in our anticoagulation clinic at least once a month.  More frequent visits may be necessary depending on the patient’s health status, medications, or lifestyle changes.

Patients can expect a personalized treatment plan at each visit, which may include:

  • One-on-one consultation with an experienced registered nurse
  • Review of medications to screen for potential drug interactions
  • A rapid test result (via fingerstick)
  • Warfarin dose adjustments, as needed
  • Education and support to reduce the occurrence of bleeding-related complications
  • Dietary counseling to prevent food interactions
  • Bridging therapy required for surgical procedures
  • Scheduling the next visit before leaving the office

To schedule an appointment, answer questions regarding Warfarin or to report any changes or upcoming procedures, please contact the ACC department at (763) 587-7856, Monday – Friday, 8:30 am – 5:00 pm.
